Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license in New York.
• Conduct home visits to assess and provide care to patients.
• Proficient in EMR documentation.
• Hold CPR Certification.
