Physical Therapist Responsibilities:

• Perform home visits for Physical Therapy assessments and direct patient care.
• Evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Utilize EMR documentation proficiently (experience with Hello Note preferred but not required).

Physical Therapist Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered PT license in New York.
• Maintain CPR certification.
