Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care to assess functional capabilities and limitations.
• Document patient information accurately using EMR systems.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements

• Graduation from an accredited Physical Therapy college or university.
• Unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
