Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program
• Hold an unencumbered Physical Therapy (PT) license for New York
• Perform home visit Physical Therapy assessments, and provide direct patient care to evaluate medical conditions, functional capabilities, limitations, and rehabilitation potential
• Proficiency in EMR documentation; experience with Hello Note is a plus
• CPR Certification
