Responsibilities
- Conduct home visit assessments and provide direct patient care.
- Assess medical conditions, functional capabilities, and potential for rehabilitation.
- Proficient in EMR documentation.
- Ensure high-quality patient care and effective therapy sessions.
Requirements
- Graduated from an accredited Physical Therapy program.
- Hold an unencumbered Physical Therapy license in New York.
- Experience in conducting home visit assessments.
- Proficiency in EMR documentation.
- CPR Certification.
