Responsibilities
- Conduct home visit Physical Therapy assessments and provide direct patient care.
- Assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
- Proficient in EMR documentation, CPR Certification.
Requirements
- Graduated from an accredited Physical Therapy (PT) college or university.
- Hold an unencumbered Physical Therapy (PT) license for New York.
- Experience in conducting home visit assessments and EMR documentation is a plus.
