Job Description

We are seeking a dedicated Registered Nurse (RN) Field Nurse for Home Care to join our team at ChristianaCare. As a Full-Time RN, you will provide skilled nursing care and treatment to geriatric patients in the community, developing patient-centered care plans and ensuring patient discharges are well planned. Our ideal candidate will be IV proficient or willing to undergo training to become so.

Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients.
• Develop and implement patient-centered care plans.
• Plan for patient discharges effectively.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of employment.
• Current DE RN license or compact state license.
• 2 years of recent HomeHealth RN experience.
• Previous Home Healthcare and Med-Surgical experience preferred.
