Rehabilitation Therapist - $70-$90 per Hour

As a Rehabilitation Therapist, you will conduct home visit assessments, provide direct patient care, and contribute to the rehabilitation process.

Responsibilities

• Conduct home visit assessments of referred patients to evaluate medical conditions and functional capabilities.
• Provide direct patient care to assist in rehabilitation efforts.
• Utilize EMR documentation systems for accurate record-keeping.
• Collaborate with patients, families, and colleagues to create personalized care plans.

Requirements

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficiency in EMR documentation; experience with Hello Note is a plus.
• Current CPR certification.
