Rehabilitation Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments, providing direct patient care, and rehabilitation potential evaluation.
• Proficient in EMR documentation.
• CPR Certification.
