Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Perform home visit assessments providing direct patient care to evaluate medical conditions and rehabilitation potential.
• Proficient in EMR documentation; experience with Hello Note is a plus.
• CPR Certification.
