Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid, unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments to evaluate patient's medical conditions and rehabilitation potential.
• Proficient in EMR documentation.
• CPR Certification.
