Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions, functional capabilities, and rehabilitation potential.
• Provide direct patient care and implement personalized treatment plans.
• Ensure accurate and timely EMR documentation.
• Collaborate with patients, families, and healthcare professionals to optimize rehabilitation outcomes.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Valid, unencumbered Physical Therapy (PT) license in New York.
• CPR certification.
• Proficiency in EMR documentation.
