Responsibilities

• Conduct home visits to perform physical therapy assessments and provide direct patient care.
• Document patient information accurately using EMR systems.
• Collaborate with patients to address medical conditions, functional capabilities, and rehabilitation potential.

Requirements

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficiency in EMR documentation preferred.
• Current CPR Certification.
