Job Description
We are looking for a dedicated and compassionate Social Worker to join our team in Baltimore County/City. In this role, you will provide professional and family-oriented services to individuals in their chosen residence, ensuring their psychosocial, environmental, and financial needs are met. You will collaborate with patients, families, and interdisciplinary team members to formulate and evaluate care plans while serving as an expert resource within the organization.
Responsibilities
- Assess patients' and families' psychosocial, environmental, and financial needs.
- Formulate, implement, and evaluate care plans in collaboration with patients, families, and interdisciplinary team members.
- Function as a consultant, educator, and expert practitioner.
- Ensure evidenced-based practice and optimal patient/family care.
- Accountability for clinical practice issues within the area of expertise.
Requirements
- Master's Degree in Social Work from an accredited school.
- LCSW certification.
- Strong interpersonal communication skills.
- Ability to maintain accurate documentation records.
