SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services, conduct psychosocial assessments, and offer counseling to patients/residents and families on a range of issues.

Responsibilities

• Provide psychosocial assessments to address various challenges faced by patients and their families.
• Offer support and counseling on adjustment to institutional living, financial concerns, family relationships, and end-of-life issues.
• Assist in discharge planning and provide guidance on community resources.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
• Ability to work collaboratively in a healthcare setting.
