SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position is PRN (as needed) at Levindale Hospital, providing clinical social work services and psychosocial assessments to patients and their families facing various challenges.

Responsibilities

• Offer support and counseling to patients/residents and families on a range of issues, including adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
• Assist in discharge planning and provide guidance related to aging and medical treatments.

Requirements

• Bachelor's degree or equivalent in social work.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
