SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a member of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Assess and address issues such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life matters.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Facilitate discharge planning and support patients and families in navigating healthcare systems.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Demonstrated knowledge and understanding of social work practices and ethics.
