SOCIAL WORKER PRN

Job Description

We are looking for a passionate PRN Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling for end-of-life matters.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families with issues related to institutional living, financial concerns, and end-of-life planning.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Participate in discharge planning and support patients through challenging transitions.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
• Compassionate and empathetic approach to patient care.
