Job Description
We are looking for a compassionate Social Worker PRN to provide clinical social work services and psychosocial assessments to patients and their families. As part of our team, you will address a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support patients with end-of-life decisions.
- Collaborate with healthcare professionals to address patient needs.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of experience in clinical social work.
- LBSW, LMSW, or LGPC certification.
