SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, families, and residents facing a variety of challenges. From adjusting to institutional living to counseling related to death and dying, you will play a crucial role in supporting individuals through difficult times.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Offer support and counseling to individuals facing medical diagnoses, aging, and end-of-life care.
• Participate in discharge planning and work collaboratively with interdisciplinary teams.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
