SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services to patients and their families, addressing a range of issues including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients through difficult transitions.
• Collaborate with healthcare professionals to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
