Job Description
We are looking for a dedicated PRN Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling and support related to medical diagnoses and treatments.
- Create discharge plans and assist with transitions.
- Address aging-related challenges and facilitate family relationships.
Requirements
- Bachelor's degree with 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Solid understanding of social work theory and practice.
