SOCIAL WORKER PRN

Job Description

We are looking for a dedicated and compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will be providing clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address common problems including adjustment to institutional living, financial concerns, family relationships, and counseling related to end-of-life issues.
• Collaborate with interdisciplinary teams to provide comprehensive care.
• Participate in discharge planning and offer support in challenging situations.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
