SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning. Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and offer counseling on various issues.
• Assist with discharge planning and provide support related to death and dying.

Requirements

• Bachelor's degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
