Job Description

We are looking for a passionate and dedicated Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning. Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common problems including adjustment to institutional living, financial concerns, and family relationships.
• Assist patients with understanding medical diagnoses, procedures, and treatments.
• Develop discharge plans and provide counseling related to death and dying.

Requirements

• Bachelor's degree and 1-3 years of experience.
• LBSW, LMSW, or LGPC certification.
• Strong working knowledge of social work theory and practice.
• Compassionate attitude and excellent communication skills.
