Job Description
We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life issues.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Offer counseling and support to individuals facing medical, emotional, and financial difficulties.
- Collaborate with healthcare teams to develop effective care plans.
- Provide discharge planning services and bereavement support.
Requirements
- Bachelor's degree and relevant professional certifications (LBSW/LMSW/LGPC).
- 1-3 years of social work experience.
- Strong knowledge of social work theory and practice.
- Compassionate and empathetic approach to patient care.
