Job Description

We are looking for a passionate PRN Social Worker to join our dedicated team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to aging and end-of-life care.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling and support for patients facing medical challenges and end-of-life care.
• Assist with discharge planning and address financial concerns.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a Social Worker.
• Licensed as an LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
