Job Description
We are currently looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health. As a social worker, you will provide critical clinical services and psychosocial assessments to patients, residents, and their families, addressing a range of emotional and practical challenges. This position offers the opportunity to make a meaningful impact on individuals facing difficult life situations.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address various challenges.
- Assist patients and families with adjustment to medical procedures and diagnoses.
- Develop discharge plans and offer support related to end-of-life care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
