Job Description: Social Worker PRN
We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges and facilitating coping mechanisms.
Responsibilities
- Offer clinical social work services to patients and families.
- Assess and address common psychosocial issues.
- Provide counseling on topics like institutional living, family relationships, aging-related changes, and end-of-life planning.
- Assist with discharge planning and financial concerns.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong interpersonal and communication skills.
