SOCIAL WORKER PRN

Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join LifeBridge Health in Baltimore, MD at Levindale Hospital. This position involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments of patients/residents and families.
• Provide counseling and support services for individuals and families facing challenges.
• Assist with discharge planning and end-of-life counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification preferred.
