SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker (PRN) to join our dedicated team at LifeBridge Health. As a Social Worker, you will provide vital clinical social work services and psychosocial assessments to patients/residents and their families, addressing a variety of common challenges they may face.

Responsibilities

• Offer psychosocial assessments and clinical social work services to patients/residents and families.
• Address issues such as adjustment to institutional living, financial concerns, family relationships, and more.
• Assist with discharge planning and provide counseling related to end-of-life issues.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience in social work.
• LBSW/LMSW/LGPC certification.
