SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges related to institutional living, financial concerns, family dynamics, medical issues, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Assist patients in adjusting to institutional living.
• Provide counseling on financial concerns and family relationships.
• Support individuals in understanding and accepting medical diagnoses and treatments.
• Participate in discharge planning and end-of-life counseling.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
