Job Summary
- Provide direct patient care services and support to patients and families.
- Coordinate an interdisciplinary team for enhanced hospice care.
- Evaluate patient needs and contribute to personalized care plans.
- Maintain patient clinical records and engage in quality improvement programs.
Requirements
- Compassionate, empathetic, and patient-centered approach.
- Current RN license or ability to obtain one.
- Minimum 1 year of nursing experience in hospice, home health, or oncology.
