Job Description
We are currently seeking a passionate and dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a full-time member of our team, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges and facilitating their well-being.
Key Responsibilities
- Conduct psychosocial assessments and provide counseling on adjustment to institutional living, financial concerns, family relationships, and end-of-life issues.
- Assist individuals in coping with medical diagnoses, discharge planning, and issues related to aging.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
- Prior experience in a hospital or healthcare setting is a plus.
