Job Description
LifeBridge Health is currently seeking a compassionate Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide vital clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support patients in difficult life transitions.
- Collaborate with healthcare teams to address patients' social and emotional needs.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
