Job Description
LifeBridge Health is seeking a compassionate Social Worker to join our dynamic team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist in discharge planning and end-of-life care discussions.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Advocate for patients' needs and facilitate access to community resources.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Experience in a hospital or healthcare setting is a plus.
