Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, families, and residents. Your responsibilities will involve addressing common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to various aspects of life changes.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Offer counseling and support for issues related to aging, medical conditions, and end-of-life planning.
- Collaborate with healthcare professionals to develop effective care plans.
- Participate in discharge planning activities.
Requirements
- Bachelor's degree in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
- Excellent communication and interpersonal skills.
