Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Execute patient-centered care plans in alignment with available resources.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
