Job Description
We are looking for a highly skilled Registered Nurse (RN) to join our Home Health team at ChristianaCare. As a Field Nurse, you will provide exceptional care to geriatric patients in the community, develop personalized care plans, and ensure optimal patient outcomes.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in a community setting.
- Develop and implement goal-oriented care plans that are patient-centered and resource-consistent.
- Create discharge plans for patients based on their individual needs.
Requirements
- BSN required; ADN candidates must work towards obtaining a Bachelors of Science in Nursing within three years of employment.
- Current DE RN license or compact state license.
- Minimum 2 years of recent Home Health RN experience.
- Experience in Home Healthcare and Med-Surgical settings preferred.
- Proficient in IV therapy or willing to undergo training.
