We are looking for a motivated Home Health Registered Nurse (RN) to join Interim HealthCare of West Palm Beach on a Per Diem basis in Vero Beach, Florida. Responsibilities

• Deliver high-quality home-based care to patients with various medical needs.
• Collaborate with a multidisciplinary team to oversee patient care and ensure positive outcomes.
• Conduct patient assessments, document progress, and communicate changes to healthcare providers.
• Educate patients and their families on care plans, medications, and wellness strategies.
• Facilitate goal achievement and coordinate patient discharge when appropriate.

Requirements

• Minimum 1 year of acute care experience; home healthcare experience is preferred.
• Active RN license in FL and CPR certification.
• Familiarity with Oasis documentation and state/federal home health regulations.
• Strong clinical skills, communication abilities, and a compassionate demeanor.
