Job Description
As a Primary Care Physician at Esperanza Health Centers, you will be responsible for providing primary medical care services to older adult participants of the Program of All-Inclusive Care for the Elderly (PACE) while emphasizing preventive care and healthy lifestyles. This role involves serving as a key member of the interdisciplinary team, coordinating care, and contributing to program planning both within the health center and the community.
Primary Duties and Responsibilities
- Manage primary care medical practice in various settings including the PACE Center, participants' homes, and post-acute facilities.
- Collaborate with the interdisciplinary team to develop individualized care plans for each participant.
- Participate in quality improvement activities and utilization management within the PACE model.
- Maintain effective communication with partner facilities and providers for seamless care coordination.
Requirements
- Bilingual proficiency in English and Spanish is essential.
- Board certification in primary care is mandatory, with a preference for certification or specialization in geriatrics.
- Valid physician license in Illinois and a driver's license with current automobile insurance.
- Prior experience working with older adults is highly preferred.
- Compliance with vaccination requirements for facilities where services are provided.
