Job Description

We are looking for a dedicated Care Team Nurse to join Pediatric Associates of Greater Salem Inc. As the clinical leader of the care team, you will work under the supervision of the Pediatrician, Nurse Practitioner, and Nurse Manager to ensure efficient operations, manage high-risk patients, provide education to patients and families, offer direct care and laboratory services, and handle telephone and direct triage effectively.

Responsibilities

  • Demonstrate infection control practices, including standard precautions.
  • Provide age-appropriate care for pediatric patients and demonstrate proficiency in operating equipment and instrumentation.
  • Assess and address the physical, emotional, and socio-economic needs of clients and families, making referrals when necessary.
  • Offer urgent care in-office and perform telephone triage/crisis management.
  • Document nursing care accurately in the EMR system, including triage, medication reconciliation, education, and care planning.
  • Facilitate communication among family members, physicians, front desk staff, and other healthcare professionals.
  • Administer vaccines, medications, conduct in-office procedures efficiently, and delegate tasks appropriately.
  • Follow up with patients post-ER or hospital discharge to prevent readmissions and complications.
  • Pursue continuing education and training in pediatric care.
  • Adhere to practice policies and procedures, meeting physical demands of the position.

Requirements

  • Strong written and verbal communication skills, excellent organizational abilities.
  • Capable of working independently and collaboratively within a care team.
  • Maintain confidentiality and HIPAA compliance with sensitive medical information.
  • Proficient in EMR, Microsoft Office, and email systems.
  • Graduation from an accredited nursing program with a current RN or LPN license in Massachusetts.
  • Hold Basic Life Support (BLS) certification and fulfill required Continuing Educational Units (CEUs).
  • Abide by the American Nursing Association Standards of Practice.
