Job Description
We are looking for an experienced Registered Nurse (RN) with a background in Home Health to join our team. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement patient-centered care plans.
- Ensure IV proficiency and participate in IV functions.
- Create discharge plans for patients based on their needs and available resources.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment.
- Current DE RN license or compact state license.
- Minimum 2 years of Home Health RN experience in the last 12 months.
- Prior Home Healthcare and Med-Surgical experience preferred.
