Job Description
We are seeking a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will collaborate with the clinical team and medical provider to provide discharge planning coordination and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, ensuring efficient health services utilization, and minimizing payment denials.
Position Details
- Sign-On Bonus Potential: $15,000
- Location: Randallstown, MD
- Department: NW Care Management
- Job Type: Full-time with Weekend Commitment (Day and Evening Shifts)
- Schedule: 4/10-hour shifts weekly, 8:30 am - 7:00 pm
Requirements
- Basic professional knowledge with a Bachelor's degree
- BSN required from CCNE accredited schools
