Job Description
We are seeking experienced Home Care LPNs to join our ChristianaCare HomeHealth team. As a dedicated LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, supporting New Castle County, DE. Join us in delivering patient-centered care and making a difference in the lives of our patients.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
