Job Description
We are looking for an experienced Licensed Practical Nurse (LPN) to join our ChristianaCare HomeHealth team. As an LPN in our organization, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in coordination with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
