Job Description
We are looking for an experienced Licensed Practical Nurse (LPN) to join ChristianaCare's HomeHealth team. As an LPN in our organization, you will provide skilled nursing care and treatment to geriatric patients in the community. You will work closely with our team to implement patient-centered care plans and support patients through their discharge process. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED is required.
- Must hold a current DE LPN license or compact state license.

$22.00 - $28.00 per hour
