We are looking for a dedicated Licensed Practical Nurse (LPN) to join ChristianaCare HomeHealth, providing exceptional care to patients in New Castle County, DE. Responsibilities
- Deliver skilled nursing care to geriatric patients in the community.
- Develop and implement patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
