Job Description
ChristianaCare HomeHealth is seeking dedicated Licensed Practical Nurses (LPNs) to join our team. If you are experienced in providing skilled nursing care in home care settings, we offer a supportive environment with extensive training opportunities.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans according to available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
