Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As part of our HomeHealth division, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to the development of patient discharge plans. This position offers a competitive hourly rate based on experience and a range of benefits including medical, dental, vision, and retirement planning services.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
